Discharge planning begins on admission with an interdisciplinary approach to ensure a seamless and safe transition back to home as quickly as possible. Upon admission, a folder is assigned to the new resident that is theirs to take home. This folder tracks their progress and stores important discharge documents that they will need upon returning to community. Our social services team also works with the resident and their families in order to schedule in home services to make the transition back to their community easier. Our number one goal is to plan for a safe and swift discharge that is aligned with the resident’s needs.
