We provide an individualized multidisciplinary approach to our rehabilitation programs that accounts for the resident’s physical and cognitive needs, as well as family feedback, to ensure that the resident receives treatment that meets our high standards of care. Union Plaza Care Center meets the diverse cultural background of our community. We can directly meet the cultural needs of the high-volume Asian American population in the surrounding community. Union Plaza Care Center is entrenched in our local community (aligned with many religious, civic, and cultural organizations) and is uniquely positioned to provide a bridge between quality healthcare and the Asian American community’s underserved needs. We have nurses, dietitians, social workers, physical and occupational therapists, speech therapists, psychological services staff, and recreation staff who are fluent in Mandarin, Cantonese, Taiwanese, Fujianese, Vietnamese, and Korean, allowing us to directly communicate with the residents and their families optimally and personally. We also cater our cuisine options for culturally appropriate diets, as well as diets geared toward spiritual needs. Residents’ spiritual needs are also catered to by our relationships with local religious leaders. We routinely accommodate religious visits and local entertainers to ensure our residents’ spiritual and activity needs are met.
Discharge planning begins on admission with an interdisciplinary approach to ensure a seamless and safe transition back to home as quickly as possible. Upon admission, a folder is assigned to the new resident that is theirs to take home. This folder tracks their progress and stores important discharge documents they will need upon returning to the community. Our social services team also works with the resident and their families to schedule in-home services to make the transition back to their community easier. Our number one goal is to plan for a safe and swift discharge aligned with the resident’s needs.
Union Plaza Care Center is able to cater to cardiovascular needs. Some specific treatments we provide include but are not limited to: CPAP/BiPAP, PICC line/medication, tracheostomy, bariatric services, and a specialized cardiac rehab program to monitor the patient’s vital signs throughout their stay. We employ a full-time, certified wound nurse to identify, treat, and contain current wounds, and to help mitigate future wounds. In addition to our full-time wound nurse, we have a wound team led by a specialized doctor who completes rounds weekly. Besides having a wound team that rounds, we have an in-house physiatrist for pain management, injections, and acupuncture treatments. Our newest addition to our facility is our secure, locked memory unit for patients with dementia. The staff on our memory unit have completed the CARES program, which is the only online training program recognized by CMS, to meet the unique needs of these residents. Our nurses are also equipped with smartphones and tablets to facilitate telemedicine.
US News and World Report rated Union Plaza Care Center as one of the top nursing homes in the country in 2021. The New York Times noted that our facility had one of the lowest pressure ulcer rates in the country. Our administrator, Dr. Pelman, is the recipient of the Administrator of the Year award in 2014.
Union Plaza Care Center never settles and is constantly striving to improve the quality of care and level of care we provide. We currently have an application pending with New York state to provide dialysis in-house to our patients, and we continue to look for opportunities to further enhance the services we offer our community.
